It will be Italy, and in particular Sardinia, at the end of the century, to count the highest dependency index in Europe. This is the ratio between the population aged at least 65 and the active population, ie aged between 15 and 64 years. This was revealed by Eurostat projections according to which, at the local level, the black jersey in Italy currently goes to Biella (with an index of 53%, in 50th place in the European Union), followed by Savona and Genoa (both with 52% ) and Trieste (51%). On the other hand, Caserta (30%), Naples (31%) and Barletta-Andria-Trani (33%).

Using the data by provinces available up to 2050, Eurostat estimates a worsening of the situation over the next 30 years throughout the Old Continent, with the exception of the Harz in Germany: the average will rise to 57% by 1 January 2050. In in particular, the highest Italian dependency index will be reached in six Sardinian provinces: Carbonia-Iglesias, Medio Campidano, Oristano, Cagliari, Ogliastra and Nuoro.

At European level, in 2021 the Greek region of Evrytania recorded the highest elderly-young ratio (78%), followed by the Belgian region of Arr. Veurne (65%) and the German region of Suhl, Kreisfreie Stadt (61%) .

The youngest in Europe remain the French outermost region of Mayotte (6%), Guyana (12%) and the Spanish region of Fuerteventura (16%).
