Record numbers were recorded in 2023 for organ donation and transplantation. And on the podium of the "most generous" there are the 30-year-olds from Sardinia, with a good 84.7% of approval. Speaking of cities, Trento is at the top of the ranking, with 78.1% of the yes votes expressed on the identity card. Sassari follows in third place and Cagliari in fifth.

These are the data that emerge from the fifth edition of the "Gift Index" report, the report of the National Transplant Center (Cnt), in view of the World Day on Donations and Tissues scheduled for Sunday 14 April , which takes stock of the declarations of willingness to donate organs and tissues registered in the registers of over 7 thousand Italian municipalities during 2023. If over the course of the past year the absolute record of donations (over 2,000) and transplants (over 4,000) was confirmed, the other New to the report is the analysis of the propensity to donate in different age groups at a regional level . The highest percentage of approval is that recorded by Sardinians between 31 and 40 years old and it is precisely this age group that excels at a national level, with an average approval of 73.8%. Following are 41-50 year olds (73.1%) and 51-60 year olds (71.3%). The results of the collection among 18-30 year olds are less good, probably because they are less informed: among the youngest the average national consensus is 68.9% while those against are 31.1%. In this demographic category the highest percentage is that of "yes" registered among young Aosta Valley people (81.7%). But it is after the age of 70 that the percentage of those who oppose donation tends to rise exponentially: the "no" are 41.5% among those aged 71-80 and as many as 55% among those over eighty. A fact probably conditioned by the erroneous belief that organ donation in old age is not possible.

To date, the Transplant Information System hosts just under 19 million registered declarations : 13.5 million yes and 5.5 million no. The Index, published in view of the National Donation Day which will be held on Sunday 14 April, is based on three indicators : percentage of declarations on electronic identity cards (Cie) issued for the adult population (weight 20/100); percentage of consents on Cie issued for adult population (20/100); percentage of consensus on the total declarations (60/100).

Among the cities with over 100 thousand inhabitants, Trento stands out for the third consecutive year, with an index of 71.07/100, a percentage of approval of 78.1% and an abstention rate of 32%. Trento precedes Verona (which rises from fourth to second place) and Sassari , with Livorno at the foot of the podium. Among the other most virtuous large cities , Cagliari is fifth , Ferrara sixth, Padua seventh, Florence eighth and Perugia ninth, Bergamo tenth. The four largest cities are in decline: Milan is 24th (it was 16th the previous year), Turin goes from 29th to 31st, Rome from 32nd to 33rd while Naples remains stuck in 39th place out of 44. Among the medium-large municipalities (among the 30 and 100 thousand inhabitants) it is once again Corato (Bari) that obtains the best result, with an index of 75.70/100, 76.2% of approval and only 15% of abstentions. The Apulian town precedes Nuoro and Gravina in Puglia (Bari).

As regards the medium-small municipalities (5-30 thousand inhabitants), in the lead is Guardiagrele in Abruzzo, in the province of Chieti (index 88.92/100, approval 97.2%, abstentions 22.5%), ahead of Leverano (Lecce) and Primiero San Martino di Castrozza (Trento). Among the small towns, those with less than 5 thousand inhabitants, Geraci Siculo (Palermo) stands out once again, the town of the Madonie where, thanks to the choice of donation by the parents of an 11-year-old girl who died in 2021, little Marta Minutella, the entire community has been involved in numerous awareness-raising activities for years. Geraci once again achieved the best result among all Italian municipalities, with an index of 95.83% and 100% of approvals: out of 163 identity cards issued in 2023, 146 yeses and no noes were collected , with only 17 abstentions. In the ranking of small towns, Rovescala (Pavia) is in second place and Cinte Tesino (Trento) is in third place.
