Blood vials with the wrong name and barcode, a mix-up that also affected the patient's identification data : these errors are believed to have caused the death of Salvatore Careddu, a man originally from San Teodoro, in Oia in September 2024. According to prosecutor Milena Aucone, Careddu died due to multiple organ failure that occurred just hours after an "erroneous" blood transfusion .

Investigations conducted by the Carabinieri of the NAS of Sassari reveal that Careddu, admitted to the emergency room of the Giovanni Paolo II hospital in Olbia, received three bags of Brh+ blood transfusions, even though his blood type was Orh+. The charge is manslaughter. A specialist and a nurse from the emergency room of the Olbia hospital, both of whom were working in the department at the time of the incident, are under investigation .

The protocol governing the transfusion process was allegedly violated. The responsibilities of those under investigation must be determined; the case certainly confirms the enormous difficulties faced by the healthcare staff in the Olbia Emergency Room, divided between managing emergencies and hospitalizations. Alessandro Scaccia, the Careddu family's attorney , stated: "Mr. Careddu's family thanks the Tempio Prosecutor's Office, represented by Dr. Milena Aucone, for the thoroughness of the investigation. Out of respect for the work awaiting the Court, I do not believe further comment is appropriate."
