On the eve of the definitive ban on paid doctors, the Region is implementing the first measures to guarantee emergency room shifts. The Regional Health Authority (ARES) has established a single regional register of private doctors for emergency care . By the first deadline, 93 applications were received, including 12 specialists, 28 residents, 51 medical graduates, and 2 retired physicians.

Meanwhile, the system of agreements between healthcare agencies is also taking shape. Arnas Brotzu and the Medio Campidano Local Health Authority (ASL) have signed an agreement that will allow Brotzu medical directors to perform additional work at ASL facilities, at a flat regional rate of €140 gross per hour.

For the USB Sanità union, the problem remains open: "Twelve specialists for twelve critical emergency rooms cannot guarantee service coverage," says regional president Gianfranco Angioni, according to whom the Region, after spending 328 million euros on fee-payers between 2024 and 2025, is replacing cooperatives with inter-company loans of doctors, failing to address the structural staff shortage.
